We work as members of the multidisciplinary team at GEH to meet the needs of those patients with a life limiting illness in order to ensure the best possible quality of life for them and those important to them.

Referrals are accepted from healthcare professionals, patients and carers with the consent of the patient and the consultant in charge of the patient’s care.

We have a weekly Specialist Palliative Care MDT Meeting which involves the hospital and community teams. 

We work closely with our colleagues in the hospital, hospice and community in supporting the holistic needs of patients and carers. This includes:

  • advice on symptom management;
  • ethical decision making;
  • signposting to other professionals or agencies;
  • providing psychological support to patients and families;
  • providing information and support with social needs;
  • providing spiritual support;
  • providing support with advance care planning;
  • assisting with rapid discharge towards the end of life;
  • supporting staff in providing care in the last days of life  
  • providing education and training for health and social care staff in palliative and end of life care.

The Consultant in Palliative Medicine provides a weekly outpatient clinic accepting referrals from hospital and community through the consultant’s secretaries (contact details above) or via the team.
