This innovative initiative aims to reduce health inequalities by delivering respiratory care directly into the community—reaching individuals who may not typically engage with mainstream healthcare services.

As part of this collaboration, CGL’s community-based drug and alcohol teams identify individuals who may be living with unmet respiratory health needs.

The hospital’s Physiological Sciences team then provides on-site spirometry and respiratory assessments, making vital diagnostics accessible to those most at risk.

The initiative’s goals are to:

  • Detect and monitor respiratory disease in underserved population
  • Support personalised and appropriate care pathways
  • Reduce barriers to accessing healthcare services

“This partnership allows us to deliver care in a truly patient-centred way,” said Joanna Purvis, Physiological Sciences Service Manager at George Eliot Hospital NHS Trust. “By working directly with CGL’s community teams, we are reaching individuals who might otherwise go undiagnosed and untreated, helping to close the gap in health outcomes.”

Change Grow Live plays a crucial role by engaging with individuals who face multiple social and health challenges. Through this collaboration, the organisations are creating a responsive, community-led model of care that meets people where they are—both literally and figuratively.

Temi Odeyemi, Change Grow Live’s Cluster Lead Nurse for Warwickshire said: “We are really excited about the partnership work with George Eliot Hospital, their offer to the people that use our service is greatly welcomed and this support will be invaluable.”

This initiative represents a significant step forward in delivering more equitable healthcare and proactive respiratory support to those most in need.
