The Sustainability and Transformation Plan (STP) is a joint agreement between NHS organisations, which shows how local health and care services will change and evolve to meet the needs of the local population.
NHS England has asked every health system to create one of these plans and George Eliot Hospital NHS Trust is part of the Coventry and Warwickshire STP.
The STP plans are not about hospitals closing. It is about designing services differently with the resources that we have and focusing on preventing physical and mental ill health. The joint vision across health and social care is to work together to deliver high quality care which supports our communities to live well, stay independent and enjoy life.
Each footprint has to be able to answer the following questions:
· How will we close the health and well being gap?
· How will we drive transformation to close the care and quality gap?
· How will we close the finance and efficiency gap?
